Discussion on the treatment of idiopathic pulmonary fibrosis accompanied by gastroesophageal reflux disease based on “acrid opening and bitter downbearing” theory

Abstract：Idiopathic pulmonary fibrosis (IPF) is a common respiratory disease, with dyspnoea after progressive exercise and dry cough as the main symptoms, often accompanied by gastroesophageal reflux disease (GERD). The pathogenes between IPF and GERD is not clear. So far, Western medicine has no good means of treatment. The main pathogenesis of traditional Chinese medicine of IPF combined with GERD is characterized by the disease of lung, spleen, stomach and kidney, the internal obstruction of dampness-heat and phlegm-stasis, the disorder of qi activity of triple energizer, and the imbalance of yin and yang. The method of acrid opening and bitter downbearing is widely used to treat the diseases of complexity of cold and heat, disorder of ascending and descending, and combination of pathogenic factors. In view of the pathogenesis of IPF combined with GERD, the method of acrid opening and bitter downbearing can be discussed and treat this disease from the point of view of appling herbs of heat and cold nature for balance of yin and yang, ascending and descending for restoring of qi activity, dispersing damp-heat for dispelling of pathogenic factors.
