Abstract:Objective To analyze the risk factors of patent ductus arteriosus with hemodynamic significance (hsPDA) in premature infants. Methods From January 2018 to June 2020, 80 cases of preterm infants with hsPDA admitted to Department of Neonatology, Xuancheng People’s Hospital, Anhui Province (“our hospital” for short) were selected as the observation group, and 80 cases of preterm infants without hsPDA delivered in our hospital during the same period were selected as the control group. The platelet (PLT) count, mean platelet volume (MPV), platelet distribution width (PDW), platelet hematocrit (PCT) and platelet large cell ratio (P-LCR) were compared between the two groups within 24 hours after birth; risk factors of hsPDA preterm infants were analyzed by logistic regression analysis. Results There were highly significant differences in gestational age, birth weight, neonatal 1 min Apgar score, neonatal respiratory distress syndrome, arterial duct diameter, left atrial diameter / aortic diameter between the two groups (all P < 0.01). There were significance differences in the ratio of cesarean section and gestational hypertension between the two groups (all P < 0.05). There were no significant differences in PLT and P-LCR between the two groups (P > 0.05); MPV, PDW and PCT in the observation group were lower than those in the control group, and the differences were highly statistically significant (all P < 0.01). Logistic regression analysis results showed that gestational age (OR = 0.721, 95%CI:0.270-0.932), cesarean section (OR = 0.601, 95%CI:0.168-0.970), gestational hypertension (OR = 0.699, 95%CI:0.231-0.807), MPV (OR = 1.669, 95%CI:1.217-2.288), PDW(OR = 1.906, 95%CI:1.460-2.488) and PCT(OR = 1.636, 95%CI:1.243-2.152) were influnce factors of hsPDA in preterm infants (P < 0.05). Conclusion In preterm infants with hsPDA, the expression of MPV, PDW and PCT are significantly reduced, the gestational age, cesarean section, gestational hypertension, MPV, PDW and PCT are independent risk factors for the disease, and have high clinical application value.
