基于Neuman理论的干预方案在子宫全切术后患者中的应用效果

摘要目的探讨基于Neuman理论为指导的护理干预应用于子宫全切术后患者心理情感状况的效果。方法选取2018年1月—6月江苏省无锡市妇幼保健院（以下简称“我院”）收治行子宫全切术的患者58例为对照组，选取2018年7月—12月我院收治行子宫全切术的患者58例为观察组。对照组采用常规护理，观察组采用基于Neuman理论的护理干预，比较两组患者焦虑、抑郁情绪状况变化。结果两组护理后焦虑自评量表（SAS）、抑郁自评量表（SDS）评分均显著低于护理前，且观察组护理后SAS、SDS评分低于对照组，差异有统计学意义（P ＜ 0.05）。两组护理后偏执、敌对、恐怖、躯体化、焦虑、强迫症状及人际关系均明显低于护理前，观察组抑郁及精神病性低于护理前；且观察组护理后恐怖、躯体化、焦虑、抑郁、精神病性、人际关系评分均低于对照组，差异有统计学意义（P ＜ 0.05）。观察组护理满意率高于对照组，差异有统计学意义（P ＜ 0.05）。结论基于Neuman理论为指导的护理干预应用于子宫全切术后患者效果显著，可有效缓解患者负性情绪，改善精神状况，提高满意度。

Abstract：Objective To explore the effect of nursing intervention based on Neuman theory in the psychological and emotional status of patients after hysterectomy. Methods A total of 58 patients who underwent hysterectomy from January to June 2018 in Wuxi Maternal and Child Health Hospital of Jiangsu Province (“our hospital” for short) were selected as the control group, while a total of 58 patients who underwent hysterectomy from July to December 2018 in our hospital were selected as the observation group. The control group used routine nursing care, and the observation group used Neuman theory-based nursing intervention to compare the changes in anxiety and depression mood of the two groups. Results The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) scores of the two groups after nursing were significantly lower than those before nursing, and the SAS and SDS scores of the observation group after nursing were lower than those of the control group, and the differences were statistically significant (P ＜ 0.05). The paranoia, hostility, horror, somatization, anxiety, obsessive-compulsive symptoms and interpersonal relationship after nursing in the two groups were significantly lower than those before nursing, the depression and psychosis of the observation group were lower than those before nursing; the observation group after nursing was horrified, somatized, anxiety, depression, psychosis, interpersonal, the relationship scores were lower than those of the control group, and the differences were statistically significant (P ＜ 0.05). The nursing satisfaction rate of the observation group was higher than that of the control group, and the difference was statistically significant (P ＜ 0.05). Conclusion Nursing intervention based on Neuman’s theory is effective when applied to patients after total hysterectomy, which can effectively alleviate patients’ negative emotions, improve mental status, and increase satisfaction.
