Impact of the “three-level linkage” elderly health management model on elderly health

Abstract:Objective To explore the effect of carrying out “three-level linkage” health management model for the elderly. Methods Using the convenience sampling method, 335 elderly inpatients in Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from February to August 2018 were selected as the research objects, and a one-year elderly health management was implemented, including health risk assessment, health risk monitoring, and health improvement. The quality of life, body composition, family function and social support of elderly patients before and after intervention were compared. Results One year after the implementation of the elderly health management model, the quality of life in addition to physiological functions, the protein, bone mineral content, skeletal muscle of body composition, family function scale and social support scale scores of elderly patients were all higher than before the intervention (P < 0.05), body weight and body fat in body composition were all lower than before intervention (P < 0.05). Conclusion The “three-level linkage” elderly health management model can effectively improve the quality of life of the elderly, improve the physiological indicators of the body, and at the same time improve the family function and social support of the elderly patients, thereby improving the health of the elderly patients.
