“三级联动”老年健康管理模式对老年人健康的影响

摘要目的开展“三级联动”老年健康管理模式，探讨其效果。方法采用便利抽样法，选取2018年2月—8月335例中国医学科学院北京协和医院老年住院患者为研究对象，实施为期1年的老年健康管理，包括评估健康风险、监测健康风险、改善健康状况。比较干预前后老年患者的生活质量、身体成分、家庭功能及社会支持。结果老年健康管理模式实施1年后，老年患者除生理职能外的生活质量，身体成分的蛋白质、骨矿物质含量、骨骼肌，家庭功能量表及社会支持量表的得分均高于干预前（P ＜ 0.05），身体成分中的体重、体脂肪均低于干预前（P ＜ 0.05）。结论 “三级联动”老年健康管理模式可有效提高老年人的生活质量、改善机体的生理指标，同时提高老年患者的家庭功能和社会支持，从而提高老年患者的健康水平。

Abstract：Objective To explore the effect of carrying out “three-level linkage” health management model for the elderly. Methods Using the convenience sampling method, 335 elderly inpatients in Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from February to August 2018 were selected as the research objects, and a one-year elderly health management was implemented, including health risk assessment, health risk monitoring, and health improvement. The quality of life, body composition, family function and social support of elderly patients before and after intervention were compared. Results One year after the implementation of the elderly health management model, the quality of life in addition to physiological functions, the protein, bone mineral content, skeletal muscle of body composition, family function scale and social support scale scores of elderly patients were all higher than before the intervention (P ＜ 0.05), body weight and body fat in body composition were all lower than before intervention (P ＜ 0.05). Conclusion The “three-level linkage” elderly health management model can effectively improve the quality of life of the elderly, improve the physiological indicators of the body, and at the same time improve the family function and social support of the elderly patients, thereby improving the health of the elderly patients.
