当归拈痛汤加减治疗糖尿病周围神经病变的效果

摘要目的观察当归拈痛汤加减治疗糖尿病周围神经病变的临床效果。方法选取2017年1月—2018年12月于中国中医科学院广安门医院内分泌科就诊的60例糖尿病周围神经病变患者，依照随机数字表法将其分为对照组和治疗组，各30例。除基础治疗外，对照组给予甲钴胺治疗，治疗组给予中药当归拈痛汤加减治疗，疗程为30 d。比较两组疗效，观察两组治疗前后空腹血糖（FPG）、餐后2 h血糖（2hPBG）、糖化血红蛋白（HbA1c）、三酰甘油与高密度脂蛋白胆固醇比值（TG/HDL-C）、正中神经和腓总神经的运动神经传导速度（MNCV）、感觉神经传导速度（SNCV）。结果治疗组的总有效率高于对照组（P ＜ 0.05）。治疗组治疗后的FPG、TG/HDL-C均低于对照组（P ＜ 0.05）；治疗组治疗后的FPG、2hPBG、HbA1c、TG/HDL-C均低于治疗前（P ＜ 0.05）；对照组治疗后的2hPBG低于治疗前（P ＜ 0.05）。两组治疗后正中神经MNCV、正中神经SNCV、腓总神经MNCV、腓总神经SNCV均快于治疗前（P ＜ 0.05）。结论当归拈痛汤加减能提高糖尿病周围神经病变患者的神经传导速度，并改善患者的临床症状，降低TG/HDL-C水平，疗效显著。

Abstract：Objective To observe clinical efficacy of Modified Danggui Niantong Decoction in the treatment of diabetic peripheral neuropathy. Methods Sixty patients with diabetic peripheral neuropathy in Department of Endocrine, Guang′anmen Hospital from January 2017 to December 2018 were selected and divided into control group and treatment group according to random number table method, 30 cases in each group. In addition to basic treatment, control group was treated with Mecobalamin, and treatment group was treated with Modified Danggui Niantong Decoction for 30 days. The curative effect of two groups was compared. FPG, 2hPBG, HbA1c, TG/HDL-C, MNCV and SNCV of median nerve and common peroneal nerve of two groups were observed before and after treatment. Results Total effective rate of treatment group was higher than that of control group (P ＜ 0.05). FPG and TG/HDL-C in treatment group after treatment were lower than those in control group (P ＜ 0.05). FPG, 2hPBG, HbA1c and TG/HDL-C in treatment group after treatment were lower than those before treatment (P ＜ 0.05). The 2hPBG of control group after treatment was lower than that before treatment (P ＜ 0.05). After treatment, the median nerve MNCV, median nerve SNCV, common peroneal nerve MNCV and common peroneal nerve SNCV of two groups were faster than those before treatment (P ＜ 0.05). Conclusion Modified Danggui Niantong Decoction can improve the nerve conduction velocity of patients with diabetic peripheral neuropathy, improve the clinical symptoms of patients, and reduce TG/HDL-C, the curative effect is significant.
