甘皂液治疗干燥综合征的临床效果及对IL-33、IL-22水平的影响

摘要目的观察甘皂液治疗干燥综合征的效果及对血清白介素（IL）-33、IL-22水平的影响。方法选取2014年12月～2016年8月苏州市中医医院风湿科收治的40例原发性干燥综合征患者，采用随机数字表法将其随机分为对照组和实验组，每组各20例。对照组给予羟氯喹治疗，实验组在此基础上加用甘皂液治疗，疗程为3个月。对比分析两组临床疗效、中医证候积分和血清IL-33、IL-22的水平变化。结果实验组临床总有效率为90%，显著高于对照组（60%），差异有统计学意义（P ＜ 0.05）。治疗后，实验组口干、眼干、胸闷、关节疼痛积分均较本组治疗前显著降低，差异有统计学意义（P ＜ 0.05）；对照组治疗后口干、眼干、关节疼痛积分较治疗前明显下降（P ＜ 0.05），但胸闷积分组内治疗前后差异无统计学意义（P ＞ 0.05）；实验组治疗后口干、眼干、胸闷及关节疼痛的证候积分均显著低于对照组（P ＜ 0.05）。两组治疗后血清中IL-33、IL-22水平均较治疗前显著下降（P ＜ 0.05），且实验组IL-33、IL-22水平显著低于对照组，差异有统计学意义（P ＜ 0.05）。结论在西医对症治疗基础上，联合甘皂液治疗干燥综合征患者是有效的，其能有效改善口眼干燥、关节疼痛、胸闷症状，以及炎性反应。

Abstract：Objective To observe the curative effect of Ganzao Liquid in the treatment of Sj■gren's syndrome and its influence on the levels of interleukin (IL)-33 and IL-22. Methods Forty patients with primary Sj■gren's syndrome admitted to Department of Rheumatism of Suzhou Hospital of Traditional Chinese Medicine from December 2014 to August 2016 were selected and randomly divided into control group and experimental group by random number table method, with 20 cases in each group. The control group was given Hydroxychloroquine, on basis of which, the experimental group was added with Ganzao Liquid. The course was three months. The clinical effect, TCM symptom scores and serum levels of IL-22, IL-33 of the two groups were compared and analyzed. Results The total effective rate of experimental group was 90%, which was higher than that of control group (60%), the difference was statistically significant (P ＜ 0.05). After treatment, the TCM symptoms scores of dry mouth, dry eye, chest distress, joint pain in the experimental group were all lower than those before treatment, the differences were statistically significant (P ＜ 0.05); the TCM symptoms scores of dry mouth, dry eye, joint pain in the control group were all lower than those before treatment (P ＜ 0.05), while there was no statistically significant difference of chest distress score in the control group before and after treatment (P ＞ 0.05); the TCM symptoms scores of dry mouth, dry eye, chest distress, joint pain in the experimental group after treatment were all lower than those of control group (P ＜ 0.05). After treatment, the levels of IL-22, IL-33 in the two groups were all lower than those before treatment (P ＜ 0.05), and the levels of IL-22, IL-33 in the experimental group were all lower than those of control group, the differences were statistically significant (P ＜ 0.05). Conclusion On basis of symptomatic treatment of western medicine, Ganzao Liquid in the treatment of Sj■gren's syndrome is effective, which can effectively improve the dry mouth, dry eye, joint pain, chest distress, and inflammatory reactions.
