Effect of micro percutaneous nephrolithotomy combined with high power holmium laser in the treatment of complex nephritic stone

Abstract:Objective To investigate the efficacy and safety of micro percutaneous nephrolithotomy combined with high power holmium laser in the treatment of complex nephritic stone. Methods 60 patients with complex nephritic stone in Chengde Central Hospital, Hebei Province from March 2015 to September 2016 were selected as research subjects. They were randomly divided into observation group (n=30) and control group (n=30) by random number table, observation group was treated with micro percutaneous nephrolithotomy combined with high power holmium laser, control group was treated with percutaneous nephrolithotomy, the clinical indexs (intraoperative blood loss, operation time, hospitalization time and postoperative time to get out of bed, stone clearance, recurrence and complications, the levels of serum creatinine (Cr) and urea nitrogen (BUN) at preoperative and postoperative 2 h, 24 h, 7 d, 30 d between two groups were compared. Results The intraoperative blood loss in observation group was less than that in control group, time of operation, the time of hospitalization and the time of getting out of bed in observation group were significantly shorter than those in control group, the differences were statistically significant (P < 0.05). The removal rate of stone in observation group (93.3%) was significantly higher than that in control group (73.3%), the recurrence rate in observation group (3.3%) was significantly lower than that in control group (20.0%), the differences were statistically significant (P < 0.05). The incidence of postoperative complications in observation group (6.6%) was significantly lower than that in control group (36.7%), the differences was statistically significant (P < 0.05). The levels of serum Cr and BUN at postoperative 24 h, 7 d, 30 d in observation group were significantly lower than those in control group, the differences were statistically significant (P < 0.05). Conclusion Micro-renal mirror combined with high-power holmium laser in the treatment of complex kidney stones is good, safe, contribute to renal function recovery. It is worthy of clinical promotion and application.
