脊柱后路术后感染危险因素分析

摘要目的探讨脊柱后路术后感染危险因素情况。方法收集2014年3月～2016年3月四川省内江市第二人民医院收治的1170例脊柱后路术患者临床资料，依据是否发生感染进行分组，感染组300例和非感染组870例。观察两组患者术前因素、术中因素、术后因素和感染率的关系情况，观察脊柱后路手术术后感染的多因素分析情况。结果感染组患者肌力≤3级、免疫功能减退、急慢性感染、手术时间≥180 min、出血量>600 mL、植异体骨、未正确使用抗生素、术前术后大小便失禁或者留置导尿时间≥3 d、术后转入ICU、留置引流管时间>5 d所占比例均低于非感染组，差异有统计学意义（P < 0.05或P < 0.01）。急性或者慢性感染、体重指数>30 kg/m2、术后转入ICU是脊柱后路术后感染的危险因素（P < 0.05）。结论急性或者慢性感染、体重指数>30 kg/m2、术后转入ICU是脊柱后路术后感染的危险因素。

Abstract：Objective To approach risk factors of postoperative infection for spinal posterior. Methods From March 2014 to March 2016, in Second People's Hospital of Neijiang City, the clinical data of 1170 spinal posterior postoperative infection patients were selected, they were divided into two groups by infection or not, infection group 300 cases and no infection group 870 cases. The preoperative risk factors, intraoperative factors, postoperative factors and infection rate of two groups were observed, the multi-factors of postoperative infection for spinal posterior were observed. Results The myodynamia≤3 grade, immunologic decline, acute or chronic infection, operation time≥180 min, amount of bleeding>600 mL, allograft, no correct using antibiotics, gatism or retention catheterization before operation, after operation ≥3 d, shift in to ICU after operation, number of drainage tube, indwelling drainage tube>5 d of infection group were lower than no infection group, the differences were statistical significance (P < 0.05 or P < 0.01). The acute or chronic infection, BMI>30 kg/m2, shift in to ICU after operation were risk factors of postoperative infection for spinal posterior (P < 0.05). Conclusion The acute or chronic infection, BMI>30 kg/m2, shift to ICU after operation are risk factors of postoperative infection for spinal posterior.
