系统性红斑狼疮患者疲劳现状及影响因素

摘要目的调查系统性红斑狼疮患者疲劳现状与影响因素。方法选取201例2017年5～7月在四川大学华西医院风湿免疫科门诊就诊的系统性红斑狼疮患者，填写自行设计的调查表、疲劳严重程度量表、焦虑自评量表、抑郁自评量表、NRS疼痛评分、匹兹堡睡眠质量评分。结果 99例（49.3%）患者有疲劳。家庭人均月收入、抑郁、疼痛是疲劳的影响因素。家庭人均月收入＞2000～5000元的患者疲劳发生率低于0～2000元患者（P ＜ 0.05），抑郁程度越重，疲劳发生率越高（P ＜ 0.01）。中度及以上疼痛患者的疲劳发生率较高（P ＜ 0.01）。结论低收入人群的疲劳状况应该得到更多关注，应采取心理学方法改善抑郁状况，采取措施减轻疼痛以减轻疲劳。

Abstract：Objective To investigate the fatigue status and the influencing factors of fatigue in systemic lupus erythematosus patients. Methods Two hundred and one patients enrolled from Rheumatic Immunology Department of West China Hospital of Sichuan University from May to July 2017 were investigated with self-designed questionnaire, fatigue severity scale, self-rating anxiety scale, self-rating depression scale, NRS pain scoring and Pittsburgh sleep quality index. Results Ninety-nine patients (49.3%) showed fatigue. Average family monthly income, depression and pain were the influencing factors. The rate of fatigue in patients with average family monthly income between 2000 to 5000 RMB was lower than patients with income less than 2000 RMB (P ＜ 0.05). The higher the degree of depression, the higher the rate of fatigue (P ＜ 0.01). Fatigue rate was higher in patients with moderate or above pain (P ＜ 0.01). Conclusion Fatigue problem of low-income people should be paid more attention. To reduce fatigue, psychological methods should be adopted to relieve depression, and measures should be taken to alleviate pain.
