ICU机械通气患者实施血栓风险评估下分级预防护理对下肢深静脉血栓作用效果

摘要目的血栓风险评估分级预防护理对重症监护室（ICU）机械通气患者下肢深静脉血栓（DVT）的作用。方法选取2020年5月至2022年5月江苏省连云港市第二人民医院ICU治疗的机械通气患者89例，以随机数字表法将其分为对照组和研究组，各45例。对照组行常规血栓预防护理干预，研究组行危险因素评估下血栓分级预防护理，比较两组护理前后血流峰值速度、血液流动学指标、凝血功能及下肢DVT发生情况。结果护理后，两组股静脉和腘静脉的血流平均速度、血流峰值速度均高于护理前，且研究组高于对照组，差异有统计学意义（P＜0.05）；两组血浆黏度、D-二聚体水平均低于护理前，且研究组低于对照组，差异有统计学意义（P＜0.05）；研究组DVT发生率低于对照组，差异有统计学意义（P＜0.05）。结论血栓风险评估下分级预防护理有助于改善血液流动学和凝血功能指标水平，降低下肢DVT的发生率。

Abstract：Objective To evaluate the effect of graded preventive nursing on lower limb deep vein thrombosis (DVT) in patients with mechanical ventilation in intensive care unit (ICU) under risk assessment of thrombosis. Methods A total of 89 patients with mechanical ventilation treated in the ICU of the Second People’s Hospital of Lianyungang, Jiangsu Province from May 2020 to May 2022 were selected, and they were divided into control group and study group by random number table method, with 45 cases in each group. The control group received routine thrombus preventive nursing intervention, and the study group received thrombus hierarchical preventive nursing under risk factor assessment. The peak blood flow velocity, blood flow index, coagulation function, and the occurrence of DVT were compared between the two groups before and after nursing. Results After nursing, the average blood flow velocity and peak blood flow velocity of femoral vein in both groups were higher than those before nursing, and the study group was higher than the control group, the differences were statistically significant (P＜0.05). Plasma viscosity and D-dimer index levels were lower than those before nursing, and the study group was lower than the control group, the differences were statistically significant (P＜0.05). The incidence of DVT in study group was lower than that in control group, and the difference was statistically significant (P＜0.05). Conclusion Graded preventive nursing under thrombus risk assessment is helpful to improve the level of hemorheology and coagulation function, and reduce the incidence of DVT in lower limbs.
