Systematic evaluation of the treatment of knee osteoarthritis by holistic bone setting and therapeutic manipulation for injured soft tissue

Abstract:Objective To systematically evaluate the efficacy and safety of holistic bone setting and therapeutic manipulation for injured soft tissue in the treatment of knee osteoarthritis (KOA) by meta-analysis. Methods PubMed, Web of Science, CNKI, Wanfang Data, and VIP were searched for the literatures evaluating the holistic bone setting and therapeutic manipulation for injured soft tissue in the treatment of KOA published from the inception to December 2022. The literatures’ information was extracted, and RevMan 5.4 was used for meta-analysis. Results A total of 12 literatures involving 922 KOA patients were included, including 464 in the treatment group and 458 in the control group. The intervention measures of the treatment group were holistic bone setting and therapeutic manipulation for injured soft tissue, and the intervention measures of the control group were local knee joint manipulation. Meta-analysis showed that the total effective rate of the treatment group was higher than that of the control group (RD=0.15, 95%CI: 0.10 to 0.20, P<0.000 01). Visual analogue scale score (MD=-0.80, 95%CI: -0.97 to -0.63, P<0.000 01), Western Ontario and McMaster University osteoarthritis pain score (MD=-1.19, 95%CI: -1.70 to -0.68, P<0.000 01), Western Ontario and McMaster University osteoarthritis stiffness score (MD=-0.61, 95%CI: -0.91 to -0.31, P<0.0001), Western Ontario and McMaster University osteoarthritis dysfunction score (MD=-6.27, 95%CI: -7.55 to -4.98, P<0.000 01) in the treatment group were lower than those in the control group. There was no significant difference in the total incidence of adverse reactions between the two groups (RD=0, 95%CI: -0.02 to 0.03, P=0.83). Conclusion Holistic bone-setting and therapeutic manipulation for injured soft tissue for KOA is superior to local manipulation for knee joint in terms of effectiveness and symptom function improvement. Limited by the number and quality of included studies, more high-quality, large-sample studies are needed to verify this.
