全科理念下连续动态随访护理在高血压合并冠心病患者中的应用效果

摘要目的探讨全科理念下连续动态随访护理在高血压合并冠心病（CHD）患者中的应用效果。方法选取2021年1月至2022年1月成都市第三人民医院收治的300例高血压合并CHD患者为研究对象，按随机数字表法将其分为常规组（采用常规护理模式）和全科组（采用全科理念下连续动态随访护理），每组150例。比较两组干预前后疾病认知程度、自我管理能力及健康生活相关行为，并记录干预期间心血管不良事件发生情况。结果干预后，两组疾病认知程度高于干预前，且全科组高于常规组（P＜0.05）；干预后，两组自我管理能力和健康生活相关行为水平高于干预前，且全科组高于常规组（P＜0.05）；全科组不良事件总发生率低于常规组（P＜0.05）。结论采取全科理念下连续动态随访护理对高血压合并CHD患者进行干预，能提高患者自我管理能力，改善健康生活相关行为，降低心血管不良事件风险。

Abstract：Objective To explore the application effect of continuous dynamic follow-up nursing under the concept of general practice in patients with hypertension and coronary heart disease (CHD). Methods A total of 300 patients with hypertension and CHD treated in the Third People’ s Hospital of Chengdu from January 2021 to January 2022 were selected as the study objects, and they were divided into routine group (routine nursing mode was used) and general practice group (continuous dynamic follow-up nursing under the concept of general practice was used) according to random number table method, with 150 cases in each group. The disease cognition degree, self-management ability, and healthy life-related behavior were compared between the two groups before and after intervention, and the occurrence of adverse cardiovascular events during intervention were recorded. Results After intervention, the scores of aspects of disease cognition degree in the two groups were higher those before intervention, and the scores in general practice group were higher than those in routine group (P＜0.05). After intervention, the self-management ability and healthy life-related behavior in both groups were higher than those before intervention, and the scores in general practice group were higher than those in routine group after intervention (P＜0.05). The total incidence rate of adverse events in general practice group was lower than that in routine group (P＜0.05). Conclusion Continuous dynamic follow-up nursing under the concept of general practice in the intervention of patients with hypertension and CHD can enhance the self-management ability, improve the healthy life-related behavior, and reduce the risk of adverse cardiovascular events.
