巨大食管胃肠道间质瘤报道1例并文献复习

摘要胃肠道间质瘤（GIST）是一类比较罕见的消化道疾病，食管GIST则更为罕见。本文回顾性分析四川省内江市第一人民医院（以下简称“我院”）收治的1例巨大食管间质瘤患者，肿瘤大小10.7 cm×7.9 cm×1.6 cm。患者以吞咽梗阻为主要症状，既往误诊为“晚期食管癌”而未能及时行手术治疗。我院胸心外科成功经右胸、腹两切口（Ivor Lewis手术）为其行手术治疗，术后病理诊断为GIST。按计划患者术后第7天正常进食并于术后第10天顺利出院。术后给予伊马替尼辅助治疗，定期复查未见复发。因此，巨大的GIST通过术前综合评估，制订个体化手术方案，术后靶向药物辅助治疗等综合治疗有助于改善GIST患者生活质量并延长其生存率。为此，本文对GIST起源、诊断、治疗进行文献复习，以期临床医生提高对本病的重视。

Abstract：Gastrointestinal stromal tumor (GIST) are a relatively rare of gastrointestinal diseases, and oesophageal GIST is even rarer. A case of giant esophageal stromal tumor admitted to the First People’s Hospital of Neijiang, Sichuan Province (hereinafter referred to as “our hospital”) was retrospectively analyzed, the tumor size was 10.7 cm×7.9 cm×1.6 cm. The main symptom of the patient was dysphagia obstruction, which was misdiagnosed as “advanced esophageal cancer” in the past and failed to receive timely surgical treatment, the patient was successfully treated by right thoracic and abdominal incision (Ivor Lewis surgery) in the Department of Cardiothoracic Surgery of our hospital. The postoperative pathological diagnosis was GIST. According to the plan, the patient ate normally on the 7th day after surgery and was discharged successfully on the 10th day after surgery. Therefore, comprehensive treatment such as preoperative comprehensive evaluation, individualized surgical plan and postoperative targeted drug-assisted therapy can help improve GIST patients’ quality of life and prolong survival rate. Therefore, this paper reviewed the literature on the origin, diagnosis, and treatment of GIST in order to improve the attention of clinicians to this disease.
