Application of umbilical artery blood gas analysis combined with ultrasonic Tei index in the diagnosis of neonatal asphyxia

Abstract:Objective To study the diagnostic value of umbilical artery blood gas analysis combined with ultrasonic Tei index in neonatal asphyxia. Methods Eighty cases of neonatal asphyxia patients from Jiangsu Shengze Hospital Affiliated to Nanjing Medical University (“our hospital”for short) from April 2019 to March 2022 were selected as asphyxia group, in the same period, 80 normal newborns of our hospital were selected as the control group. Umbilical artery blood gas indexes (hydrogenion concentration [pH], base excess (BE), arterial carbon dioxide pressure (PaCO2)], and ultrasonic Tei index were compared between two groups; the influencing factors of neonatal asphyxia were analyzed by logistic regression; and the diagnostic value of umbilical artery blood gas indexes combined with ultrasonic Tei index in neonatal asphyxia was determined by receiver operating characteristic curve. Results There were statistically significant differences in anemia during pregnancy, Apgar score 1 min after birth, oligohydramnios, umbilical cord around neck, and intrauterine distress between two groups (P<0.05). The pH and BE of asphyxia group were lower than those of control group, PaCO2 and ultrasonic Tei index were higher than those of control group (P<0.05). Oligohydramnios, intrauterine distress, pH, BE, PaCO2, and ultrasonic Tei index were the influencing factors of neonatal asphyxia (P< 0.05). The area under the curve of pH, BE, PaCO2, and ultrasonic Tei index combined detection was higher than that of each single detection. Conclusion Umbilical artery blood gas analysis combined with ultrasonic Tei index has high diagnostic value for neonatal asphyxia, and has certain clinical guiding significance.
