Application effect of rectal magnetic resonance imaging combined with endorectal ultrasound in diagnosis of complete response after neoadjuvant chemotherapy for rectal cancer

Abstract:Objective To investigate the application effect of rectal magnetic resonance imaging (MRI) combined with endorectal ultrasound (ERUS) in diagnosis of complete response after neoadjuvant chemotherapy for rectal cancer. Methods A total of 66 patients with rectal cancer who received neoadjuvant chemotherapy and radical surgery in the First Affiliated Hospital of Hebei North University from January 2020 to December 2021 were selected. Rectal MRI and ERUS examinations were performed before and the seventh week after neoadjuvant chemotherapy to evaluate whether the patients had complete response. Pathological examination was used as the gold standard. The consistency of different tests in diagnosing complete response were compared with the gold standard. Results Pathological results showed that 22 of 66 patients with rectal cancer achieved complete response after neoadjuvant chemotherapy. There was no statistical significance between MRI and ERUS in the diagnosis of complete response after neoadjuvant chemotherapy and the gold standard (P>0.05), while MRI combined with ERUS was consistent with the gold standard (Kappa=0.400, P<0.05). The accuracy, specificity, and sensitivity of MRI combined with ERUS in the diagnosis of complete response after neoadjuvant chemotherapy in rectal cancer patients were 75.76%, 90.91%, and 45.45%. Conclusion After neoadjuvant chemotherapy for rectal cancer, the accuracy of simple MRI and ERUS in the diagnosis of complete response is not high, and the combined diagnosis has higher specificity and lower sensitivity than single diagnosis. For patients with non-complete response, radical surgery is simply performed. For patients with complete response, colonoscopy and digital examination are recommended, and subsequent treatment is implemented with caution.
