Research on the law of traditional Chinese medicine in the differentiation and treatment of apoplexy based on ancient and modern medical cases

Abstract:Objective To mine the data of ancient and modern medical cases of apoplexy by using statistical analysis and social network analysis, and to analyze the law of their identification and treatment of apoplexy. Methods The medical case data were retrieved, collected, and organized from the super star electronic book database, and the retrieval period was from inception to December 2022, while the medical case data were screened and extracted in conjunction with the National Joint Catalogue of Chinese Medicine Books, and statistical analysis and data mining were performed on the high incidence syndrome type, nature and flavour of traditional Chinese medicine, channel tropism of apoplexy and so on. Results A total of 404 medical cases of apoplexy were included, involving 48 medical case collections, 340 Chinese herbal medicines, 4 937 total medication frequencies, three high frequency symptom types (syndrome of blood stasis due to qi deficiency, syndrome of wind-phlegm obstructing collateral, syndrome of stirring wind due to yin deficiency). The drug natures were usually warm, plain, and cold. The drug flavours were mainly sweet, bitter, and pungent. The drug channel tropisms were liver, spleen, and heart channels. There were ten high centrality value drugs. The results of the social network analysis showed that there was a strong association between the type of symptoms and the formulas and herbal medicines. The treatment was mainly composed of Buyang Huanwu Decoction, Dihuang Yinzi, and other prescriptions. Conclusion Ancient and modern medical cases suggest that apoplexy is closely related to the liver, spleen, and heart. Syndrome of blood stasis due to qi deficiency, syndrome of wind-phlegm obstructing collateral, syndrome of stirring wind due to yin deficiency are the core syndrome types. There is an obvious trend of concentration in the use of drugs, such as invigorating qi, promoting blood circulation for removing blood stasis, and suppressing hyperactive liver for calming endogenous wind, which can provide certain reference for clinical prescription drug use.
