Abstract:Objective To investigate the diagnostic value of ultrasound scoring method combined with plasma human placental prolactin (hPL) level for placenta previa combined with placenta accreta. Methods A total of 128 patients with placenta previa who were admitted to the Second People’s Hospital of Hengshui, Hebei Province from September 2019 to October 2021 were selected as the research subjects. According to the pathological results, they were divided into implanted group and non-implanted group. The clinical data, ultrasound scores, and plasma hPL mRNA levels of patients were compared between implanted group and non-implanted group. Receiver operating characteristic curve was drawn to evaluate the diagnostic performance of ultrasound scoring method, plasma hPL mRNA level and their combination detected in the diagnosis of placenta previa combined with placenta accreta by area under the curve (AUC). Results There were 68 cases of placenta previa combined with placenta implantation in 128 cases. The ultrasonic score, plasma hPL mRNA level of implanted group was higher than that of non-implanted group (P<0.05). The proportion of placental tissue thickness ≥3 cm, the proportion of postplacental hypogyrus vocal cord deletion, the proportion of the thinnest myometrium thickness<1 mm, the proportion of rough bladder surface, the proportion of increased blood flow at the base of the placenta, and the proportion of blood flow signal in cervix and surrounding of implantation group were higher than those in non-implanted group (P<0.05). Ultrasound score method and plasma hPL mRNA level were the influencing factors of patients with placenta accreta combined with placenta previa (OR>1, P<0.05). The AUC of ultrasound scoring method combined with plasma hPL mRNA level in the diagnosis of placenta previa combined with placenta accreta was higher than that of ultrasound scoring method and plasma hPL mRNA level alone (P<0.05). Conclusion Ultrasound scoring method combined with plasma hPL mRNA level have high diagnostic performance for placenta previa combined with placenta accreta.
