Abstract:Objective To investigate and intervene the non-rational use of cephalosporins situation in medical orders of inpatients in the Second Hospital of Jinhua, Zhejiang Province (hereinafter referred to as “our hospital”). Methods A total of 1 550 medical orders of cephalosporins issued to inpatients in the internal medicine ward of our hospital from January to December 2020 were selected for investigation, 750 medical orders from January to June 2020 were evaluated according to relevant standards, and 800 medical orders were intervened from July to December 2020. Relevant indicators in the doctor’s orders and the use of cephalosporins before and after intervention were counted, and the situation of non-rational use of drug before and after intervention was compared. Results Before and after intervention, the average number of drug varieties prescribed by doctors, the percentage of antibiotics prescribed by doctors, the proportion of cephalosporins used, the average amount of medical orders, and the proportion of cephalosporins prescribed by doctors were all in a stable and normal range. Most cephalosporins were Cefoperazone Sodium Sulbactam Sodium (41.94%), Ceftazidime (22.58%), Cefuroxime Sodium Powder Needle (9.67%), and Cefuroxime Ester Tablets (9.67%). After intervention, the incidence of unsuitable indications, unsuitable drug selection, unsuitable drug dosage form or route of administration, and unsuitable drug combination were all lower than those before intervention (P<0.05). The reasons for irrational drug use were mainly the lack of pharmacological knowledge of physicians and patients’ failure to follow the doctor’s advice. Conclusion Medical orders for cephalosporins in our hospital basically meet the standards, and non-rational use of drug is less. In the future, we should continue to strengthen the supervision of medical orders, conduct regular drug use investigation, and supervise rational drug use, so as to reduce the incidence of irrational drug use and provide guarantee for the safety of drug use for the majority of patients.
