Distribution of clinical symptoms and syndrome in enthyroid Hashimoto thyroiditis patients

Abstract:Objective To explore the distribution of clinical symptoms and syndrome in enthyroid hashimoto thyroiditis (HT) patients. Methods Enthyroid HT patients treated in the Department of Thyroid, Dongzhimen Hospital and Sun Simiao Hospital of Beijing University of Chinese Medicine from December 2020 to December 2021 were selected, the four diagnosis information of them was collected by traditional Chinese medicine symptom scale, and statistical analysis and cluster analysis were carried out by SPSS software. Results A total of 267 cases of enthyroid HT were collected in this study. After standardizing the name, 52 traditional Chinese medicine symptoms and signs with a frequency of more than 10% were screened. The most common symptoms were irritability (73.03%), fatigue (69.66%), memory loss (62.17%), anxiety (61.80%), pharyngeal foreign body sensation (54.68%), dry mouth (51.31%), palpitation (50.56%), depression and sadness (50.19%), the most common sign was goiter (68.16%); eight tongue signs with frequency greater than 10% were screened, the most common tongue signs were white fur (71.16%), thin fur (61.05%) and teeth-printed tongue (31.09%); three pulse types with frequency greater than 10% were screened,the most common pulse was stringy pulse (34.83%). After cluster analysis, three traditional Chinese medicine syndrome types were obtained: stagnation of liver qi, phlegm-dampness due to spleen deficiency; Stagnation of heat in liver channel, deficiency of spleen and phlegm coagulation; deficiency of spleen and kidney, syndrome of deficiency of both yin and Yang. Conclusion This study shows that enthyroid Hashimoto thyroiditis can be divided into three types: stagnation of liver qi, phlegm-dampness due to spleen deficiency at the beginning; the middle stage of the disease is stagnation of liver qi, syndrome of phlegm-dampness due to spleen deficiency; the disease is characterized by deficiency of spleen and syndrome of kidney and deficiency of both yin and Yang.
