院内至出院高危儿随访延伸服务在早产儿护理管理中的应用

摘要目的探讨早产儿护理管理中行院内至出院高危儿随访延伸服务的措施及效果。方法选取青岛市市立医院儿科在2020年1月至10月收治的100例早产儿，采用随机数字表法分为对照组及观察组，每组50例。对照组行常规护理干预，观察组在常规护理基础上再行院内至出院高危儿随访延伸服务。观察护理干预后患儿体格指标变化、神经发育状况及神经系统后遗症发生情况。结果出院后6个月，两组身高、体重高于出院时，且观察组高于对照组，差异有统计学意义（P ＜ 0.05）。干预后，两组行为能力、主动肌张力、被动肌张力、一般估价、原始反射及总分高于干预前，且观察组高于对照组，差异有统计学意义（P ＜ 0.05）。观察组出院后6个月神经系统后遗症总发生率低于对照组，差异有统计学意义（P ＜ 0.05）。结论早产儿护理管理中引入院内至出院高危儿随访延伸服务，能够促进早产儿体格及神经功能更好发育，进而提高其总体康复效果。

Abstract：Objective To explore the measures and effects of the extended follow-up service for high-risk infants from in-hospital to discharge in the nursing management of premature infants. Methods A total of 100 premature infants admitted to the Department of Pediatrics of Qingdao Municipal Hospital from January to October 2020 were selected and divided into control group and observation group by random number table method, with 50 cases in each group. The control group was received routine nursing intervention, while the observation group was received extended follow-up services for high-risk children from in-hospital to discharge on the basis of routine nursing. The changes of physical indicators, neurodevelopmental status and the occurrence of neurological sequelae in the children after nursing intervention were observed. Results Six months after discharge, the height and weight of the two groups were higher than those at the time of discharge, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P ＜ 0.05). After the intervention, the behavioral ability, active muscle tone, passive muscle tone, general assessment, primitive reflex, and total score of the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P ＜ 0.05). The total incidence of neurological sequelae six months after discharge in the observation group was lower than that in the control group, and the difference was statistically significant (P ＜ 0.05). Conclusion The introduction of extended follow-up services for high-risk infants from in-hospital to discharge in the nursing management of preterm infants can promote the better physical and neurological development of preterm infants, thereby improving their overall rehabilitation effect.
