Application value of two-dimensional ultrasound combined with four-dimensional ultrasound spatio-temporal image correlation in the screening of congenital heart disease in the second fetus#br#

Abstract:Objective To investigate the application value of two-dimensional ultrasonic combined with four-dimensional ultrasonic spatio-temporal image correlation (STIC) in the screening of congenital heart disease in the second fetus. Methods A total of 260 pregnant women with the second fetus and suspected fetal congenital heart disease who underwent prenatal screening in Hebei Provincial People’s Hospital and Dingzhou People’s Hospital, Hebei Province, from January 2019 to December 2020 were selected, and all of them underwent two-dimensional ultrasonic and four-dimensional ultrasonic STIC examination during prenatal screening. Autopsy or neonatal echocardiogram results were used as the gold standard, the detection rate and diagnostic efficacy of two-dimensional ultrasound, four-dimensional ultrasonic STIC, and their combination for congenital heart disease were compared. Results Congenital heart disease was detected in 63 of the 260 newborns, 45 of which were detected by two-dimensional ultrasound, 52 of which were detected by four-dimensional ultrasonic STIC, and 62 cases were detected by the combination of the two. There was a high consistency between two-dimensional ultrasound and gold standard in the diagnosis of congenital heart disease in the second fetus (P < 0.05). The four-dimensional ultrasonic STIC and its combination with two-dimensional ultrasound were almost identical with the gold standard in the diagnosis of congenital heart disease in the second fetus (P < 0.05). Two-dimensional ultrasound, four-dimensional ultrasonic STIC, and their combination all had certain identification ability in screening for congenital heart disease in the second fetus, the sensitivity and specificity of their combination were 98.41% and 100.00%, respectively. Conclusion The diagnostic results of two-dimensional ultrasound combined with four-dimensional ultrasonic STIC for congenital heart disease in the second fetus are almost identical with the gold standard, which can provide a basis for precise prenatal screening.
