基于怀旧主题的叙事疗法在晚期肿瘤患者心理干预中的应用

摘要目的探讨基于怀旧主题的叙事疗法在晚期肿瘤患者心理干预中的应用。方法选择2018年5月至2019年8月南京医科大学附属淮安第一医院收治的晚期肿瘤患者140例，按照随机数字表法分为干预组70例、对照组70例。对照组给予常规健康教育及心理干预，干预组联合应用基于怀旧主题的叙事疗法。干预8周后，采用焦虑自评量表（SAS）、抑郁自评量表（SDS）、状态自尊量表（SSES）、中文版Herth希望量表（HHI）评估心理状态、自尊水平、希望水平。结果干预8周后，干预组脱落2例，对照组脱落6例。两组SAS、SDS评分均低于干预前，且干预组SAS、SDS评分低于对照组（P ＜ 0.05）；两组SSES评分均高于干预前；干预组外表自尊、行为自尊、SSES总分均高于对照组（P ＜ 0.05）；两组HHI评分均高于干预前；且干预组HHI评分高于对照组（P ＜ 0.05）。结论基于怀旧主题的叙事疗法有助于缓解晚期肿瘤患者负性情绪，唤醒自尊感，提高生活希望水平。

Abstract：Objective To explore the application of narrative therapy based on nostalgia theme in psychological intervention of patients with advanced cancer. Methods A total of 140 patients with advanced cancer admitted to Huaian First Hospital Affiliated to Nanjing Medical University from May 2018 to August 2019 were selected. According to the random number table method, they were divided into 70 cases in the intervention group and 70 cases in the control group. The control group was given regular health education and psychological intervention, and the intervention group was combined with narrative therapy based on the theme of nostalgia. After eight weeks of intervention, the self-rating anxiety scale (SAS), self-rating depression scale (SDS), state self-esteem scale (SSES), and the Chinese version of Herth hope scale (HHI) were used to assess mental state, and levels of self-esteem and hope. Results After eight weeks of intervention, two cases fell off in the intervention group and six cases fell off in the control group, and the SAS and SDS scores of the two groups were lower than those before the intervention, and the SAS and SDS scores of the intervention group were lower than those of the control group (P ＜ 0.05); the SSES scores of the two groups were higher than those before the intervention; the total scores of appearance self-esteem, behavioral self-esteem, and SSES of the intervention group were higher than those of the control group (P ＜ 0.05); the HHI scores of the two groups were higher than those before the intervention, and the HHI scores of the intervention group were higher than those of the control group (P ＜ 0.05). Conclusion Narrative therapy based on nostalgia theme can help relieve the negative emotions of patients with advanced cancer, awaken their sense of self-esteem, and improve the level of life hope.
