Predictive value of total bilirubin and its ratio to direct bilirubin in the prognosis of patients with extensive-stage small cell lung cancer#br#

Abstract:Objective To evaluate the predictive value of total bilirubin to direct bilirubin and the ratio of the two in the prognosis of patients with extensive-stage small cell lung cancer (ES-SCLC). Methods A total of 83 ES-SCLC patients admitted to the 901st Hospital of the Joint Service Support Force of the Chinese People’s Liberation Army from January 2018 to July 2020 were selected as the research objects. Clinical data, biochemical tests, progression-free survival (PFS) and overall survival (OS) of patients upon admission were collected. Multivariate Cox regression was used to analyze the effect of bilirubin on the survival and prognosis of ES-SCLC patients, and the Kaplan-Meier method was used to evaluate the survival time. Results Total bilirubin and total bilirubin / direct bilirubin had predictive value for the survival and prognosis of ES-SCLC patients (P < 0.05), and the best cut-off values were 12.3 μmol/L and 2.8, respectively; compared with patients in the low total bilirubin group (≤12.3 μmol/L), patients in the high total bilirubin group (>12.3 μmol/L) had longer median PFS and OS (P < 0.05); compared with patients in the low total bilirubin / direct bilirubin group (≤2.8), patients in the high total bilirubin / direct bilirubin group (>2.8) had longer median PFS and OS (P < 0.05); total bilirubin and total bilirubin / direct bilirubin were independent prognostic factors affecting ES-SCLC patients (P < 0.05). Conclusion Total bilirubin level and total bilirubin / direct bilirubin level may be important prognostic factors of ES-SCLC, which can help determine the prognosis of patients.
