Influential factors of postoperative recurrence of chronic sinusitis with nasal polyp and the diagnostic value of peripheral blood eosinophil combined with CD4+/CD8+#br#

Abstract:Objective To investigate the influential factors of postoperative recurrence of chronic sinusitis with nasal polyp and the diagnostic value of peripheral blood eosinophil (EOS) combined with CD4+/CD8+. Methods A total of 142 patients with chronic sinusitis and nasal polyp who underwent transnasal endoscopic surgery in Wuxi People’s Hospital of Jiangsu Province from January 2018 to March 2020 were selected as the research subjects. According to their recurrence, patients were divided into recurrence group (84 cases) and non-recurrence group (58 cases). Clinical data and biochemical indicators of patients were collected, and logistic regression model was used to analyze the factors influencing postoperative recurrence. The diagnostic value of peripheral blood EOS combined with CD4+/CD8+ detection for postoperative recurrence of chronic sinusitis with nasal polyp was analyzed by receiver operator characteristic curve and the area under the curve (AUC). Results The Lund-Kennedy score, neutrophil count, neutrophil to lymphocyte ratio (NLR), and percentage of EOS in peripheral blood in the recurrence group were higher than those in the non-recurrence group, and CD4+/CD8+ was lower than that in the non-recurrence group, the differences were statistically significant (P < 0.05). Multivariate analysis showed that high NLR and high percentage of EOS in peripheral blood were independent risk factors for postoperative recurrence of chronic sinusitis with nasal polyp (OR > 1, P < 0.05), and high CD4+/CD8+ was protective factor for postoperative recurrence of chronic sinusitis with nasal polyp (OR < 1, P < 0.05). Both the percentage of EOS and CD4+/CD8+ in peripheral blood and their combination had diagnostic value for postoperative recurrence of chronic sinusitis with nasal polyp (AUC > 0.5, P < 0.05), and AUC of the combined diagnosis was larger than that of the single diagnosis (P < 0.05). Conclusion The percentage of EOS and CD4+/CD8+ in peripheral blood are the influential factors of postoperative recurrence of chronic sinusitis with nasal polyp, which have higher diagnostic value.
