行为干预联合心理护理对住院精神分裂症患者疗效研究

摘要目的探讨行为干预联合心理护理对首次住院精神分裂症患者的影响。方法选择2013年12月～2014年12月上海市精神卫生中心接受治疗的74例精神分裂症患者作为研究对象，按照随机数字表法分为观察组和对照组，每组各37例。对照组患者给予常规精神科护理，观察组患者则在对照组的基础上实施行为训练联合心理护理进行干预，干预周期为8周。采用自知力与治疗态度问卷（ITAQ）对两组患者的认识及治疗态度进行比较，采用简明精神病评定量表（BPRS）评定两组患者治疗前后的精神症状，采用大体评定量表（GAS）测定治疗前后病情严重程度。结果干预前，两组患者的ITAQ评分、BPRS评分、GAS评分差异无统计学意义（P > 0.05）；干预后，两组患者的ITAQ评分、GAS评分明显高于干预前，且观察组明显高于对照组，差异有统计学意义（P < 0.05）；两组患者BPRS评分明显低于干预前，且观察组明显低于对照组，差异有统计学意义（P < 0.05）。结论行为干预联合心理护理有利于改善住院精神分裂症患者的不良情绪，并在一定程度上减轻了病症，提高了大体功能水平，因而值得推广。

Abstract：Objective To explore the effect of behavioral intervention combined with psychological nursing on the first-episode schizophrenia patients. Methods From December 2013 to December 2014, in Shanghai Mental Health Center, 74 schizophrenic patients were selected as the research object. According to the random number table, they were divided into observation group and control group, with 37 cases in each group. Patients in the control group were treated with risperidone only, while the observation group was treated with behavioral training combined with psychological nursing intervention on the basis of the control group, the intervention period was 2 months. The insight and treatment attitude questionnaire (ITAQ) was used to compare the cognition and attitude towards the treatment of patients in the two groups, the Brief Psychiatric Rating Scale (BPRS) was used to evaluate psychiatric symptoms in two groups before and after treatment, the global assessment scale (GAS) was used to measure the severity of the disease before and after treatment. Results Before treatment, there was no significant difference in ITAQ, BPRS and GAS scores of patients in the two groups (P > 0.05). After treatment, ITAQ, BPRS scores in two groups were higher than before treatment, and ITAQ, GAS scores in observation group were higher than control group, the differences were statistically significant (P < 0.05); BPRS scores were lower than before treatmnet, and BPRS scores in observation group were lower than control group, the differences were statistically significant (P < 0.05). Conclusion The behavioral intervention combined with psychological nursing intervention can improve the negative emotions of schizophrenic patients, alleviate symptoms to some extent, improve the level of general function, thus worthy of promotion.
