Abstract：Objective To discuss the prescription of irrational Chinese patent medicine in outpatient department and put forward measures. Methods Based on Hospital prescription review management standard, Clinical application guidelines of Chinese patent medicine and the drug instructions. The prescriptions of irrational Chinese patent medicine in the outpatient from July 2016 to July 2017 were classified and analyzed. Results The irrational use of drugs was summed up as follows: inconsistent of diagnosis and prescriptions, unreasonable of combination, no signature of doctor, usage error, wrong prescription color, ultra-maximum dosage of instructions, inappropriate dosage of children, extraordinary prescription, contraindications, ultra-limit and repeated medication. The larger proportion departments in unreasonable use of drugs were mainly TCM department, hand surgery department, cardiology, orthopaedic orthopaedics, cadre branch and so on. Conclusion The pre-audit intervention of prescription is of great significance to the safety of patients, the doctors and pharmacists should constantly improve the level of knowledge about traditional Chinese medicine, measures of unreasonable prescription surveillance should improve and regulate the rational use of traditional Chinese medicine.
