Abstract:Objective To investigate the relationship between the expression of miR-125a-3p and miR-224-3p in serum and carotid atherosclerotic (CAS) plaque in cerebral infarction. Methods A total of 120 patients with cerebral infarction admitted to Beijing Hepingli Hospital from March 2019 to December 2021 were selected and divided into unstable group (75 cases) and stable group (45 cases) according to their CAS plaque stability. Clinical data of the two groups were collected and compared, and the expression of miR-125a-3p and miR-224-3p in serum of the two groups was detected by real-time fluorescent quantitative PCR. The influencing factors of CAS instability in patients with cerebral infarction were analyzed by logistic regression model, and the predictive value of CAS plaque plaque stability in patients with cerebral infarction was further analyzed by receiver operation characteristic (ROC) curve. Results The proportion of smokers, the proportion of diabetic mellitus patients, the level of low-density lipoprotein cholesterol (LDL-C), and the expression of miR-125a-3p in unstable group were higher than those in stable group, and the expression of miR-224-3p was lower than that in stable group, the differences were statistically significant (P<0.05). Multivariate analysis showed that smoking, diabetes mellitus, LDL-C, miR-125a-3p, and miR-224-3p were all influencing factors of CAS plaque instability in patients with cerebral infarction (P<0.05). The area under the ROC curve of the five items combined to predict CAS plaque instability in patients with cerebral infarction was 0.915, and the sensitivity and specificity were 84.00% and 91.11%, respectively. Conclusion Serum miR-125a-3p and miR-224-3p levels in patients with cerebral infarction are associated with CAS plaque instability, which can be predicted by combining smoking, diabetes mellitus, and LDL-C.
