Abstract:Objective To investigate the relationship between serum microRNA-137 (miR-137) and Notch receptor 1 (Notch1) expression with diabetic retinopathy (DR). Methods A total of 147 type 2 diabetes mellitus (T2DM) patients admitted to the First Central Hospital of Baoding, Hebei Province (hereinafter referred to as “our hospital”) from January 2021 to December 2022 were selected as T2DM group, which were divided into 45 patients in the DR group and 102 patients in the non-DR group according to whether they had concurrent DR, and 15 patients in the hyperplastic stage group, and 30 patients in the non-hyperplastic stage group according to the DR stage. Another 52 healthy subjects in our hospital during the same period were selected as the control group. Clinical data of T2DM patients were collected, and serum miR-137 and Notch1 mRNA expressions were detected by real-time fluorescence quantitative polymerase chain reaction. Pearson correlation coefficient was used to analyze the correlation between serum miR-137 and Notch1 mRNA expression in patients with DR, and the influencing factors for the occurrence of patients with T2DM complicated with DR were analyzed. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum miR-137, and Notch1 mRNA expression in patients with T2DM. Results Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the T2DM group compared with the control group (P<0.05). Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the DR group compared with the non-DR group (P<0.05). Serum miR-137 was negatively correlated with Notch1 mRNA expression in DR patients (r=-0.780, P<0.001). Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the proliferative group compared with the non-proliferative group (P<0.05). Multifactorial logistic regression analysis showed that prolonged T2DM disease, elevated glocosglated hemoglobin albumin, and miR-137 ≥1.47 were independent risk factors for patients with T2DM complicated with DR, and Notch1 mRNA ≥0.98 was an independent protective factor[OR (95%CI)=1.181 (1.059-1.318), 1.613 (1.170-2.224), 3.550 (0.734-0.958), and 0.188 (0.071-0.498)]. ROC curve analysis showed that the area under the curve of serum miR-137 and Notch1 mRNA expression combined to predict concurrent DR in T2DM patients was greater than that predicted by miR-137 and Notch1 mRNA alone (P<0.05). Conclusion High serum miR-137 expression and low Notch1 expression in T2DM patients are closely associated with DR and may be an auxiliary predictor of DR.
