Abstract:Objective To compare the effect of mini open and minimally invasive transforaminal lumbar interbody fusion in the treatment of lumbar degenerative diseases. Methods A total of 84 patients with lumbar degenerative diseases admitted to the 904 Hospital of PLA Joint Logistic Support Force from June 2014 to December 2020 were prospectively selected and divided into control group and study group according to the random number table method, with 42 cases in each group. The study group was treated with minimally invasive transforaminal lumbar interbody fusion, while the control group was treated with mini open transforaminal lumbar interbody fusion. The patients were followed up for one year. The operation conditions of the two groups were compared, and the lumbar function and anatomical indexes before and one year after operation were statistically analyzed. The adjacent segment imaging degeneration and facet joint intrusion of the two groups were recorded one year after operation. Results The operation time of study group was longer than that of control group (P < 0.05). There were significant differences in intraoperative blood loss and postoperative drainage volume between the two groups (P > 0.05). At one year after operation, the Japanese Orthopaedic Association (JOA) score, fuse segmental angles, and intervertebral space height of two groups were higher than those before operation, and the Oswestry disability index (ODI) and intervertebral motion of two groups were lower than those before operation (P < 0.05). There were no significant differences in JOA score, ODI, fusion segment angle, intervertebral space height, and intervertebral motion between the two groups before and one year after operation (P > 0.05). The incidence of adjacent segment imaging degeneration in the study group was lower than that in the control group (P < 0.05). There was no significant difference in the degree of facet joint intrusion of left and right side of L4 superstratum between the two groups (P > 0.05). Conclusion The efficacy of mini open transforaminal lumbar interbody fusion is comparable to that of minimally invasive transforaminal lumbar interbody fusion in the treatment of lumbar degenerative diseases, but the operation time of mini open transforaminal lumbar interbody fusion is shorter, and the risk of early adjacent segment imaging degeneration after minimally invasive transforaminal lumbar interbody fusion is lower.
