椎间盘退变中阴阳理论阐释及其临床意义

摘要椎间盘退变是一个复杂的过程，目前认为是多因素引起的椎间盘内环境中分解与合成代谢的动态平衡被打破，最终导致髓核细胞的凋亡。这个复杂的过程又与阴阳理论中“阴阳失衡”所致的疾病观点不谋而合。椎间盘退变中动态、血管减少、破骨细胞增多、炎症因子、分解代谢、细胞凋亡等属于阳，静态、血管增生、炎症因子抑制酶、合成代谢、细胞生长等属于阴。椎间盘退变的阴阳理论阐释可发展中医微观辨证理论，指导临床用药及功能锻炼。现代研究则指导多靶点平衡的机制探讨及其疾病多方面综合因素的考虑。从阴阳理论探析椎间盘退变的机制，构建中医理论与微观结构结合的桥梁，为椎间盘退变疾病的诊治提供新的思路。

Abstract：Intervertebral disc degeneration is a complex process. At present, it is considered that the dynamic balance of catabolism and anabolism in the intervertebral disc environment is broken by many factors, which eventually leads to the apoptosis of nucleus pulposus cells. This complicated process coincides with the view of disease caused by “imbalance of yin and yang” in the theory of yin and yang. In the process of intervertebral disc degeneration, dynamic state, decreased blood vessels, increased osteoclasts, inflammatory factors, catabolism and apoptosis belong to yang, while static state, vascular proliferation, inflammatory factor inhibitory enzyme, anabolism and cell growth belong to yin. The explanation of yin and yang theory of intervertebral disc degeneration can develop the micro syndrome differentiation theory of traditional Chinese medicine, guiding clinical medication and functional exercise. Modern research guides the exploration of the mechanism of multi-target balance and the consideration of various factors of disease. This paper analyzes the mechanism of intervertebral disc degeneration from the theory of yin and yang, and constructs a bridge between the theory of traditional Chinese medicine and microstructure, which provides a new idea for the diagnosis and treatment of intervertebral disc degeneration.
