Influence of chronic disease management led by clinical pharmacists on the therapeutic effect of patients with diabetes mellitus type 2

Abstract:Objective To explore the influence of chronic disease management led by clinical pharmacists on the therapeutic effect of patients with diabetes mellitus type 2. Methods A total of 132 patients with diabetes mellitus type 2 admitted to the Third Hospital of Hebei Medical University from January 2018 to December 2019 were selected. According to random number table method, they were divided into control group (66 cases) and study group (66 cases). The control group was given traditional medical services, the study group was given chronic disease management services led by clinical pharmacists. The intervention lasted for six moths, the main indicators of diabetes mellitus type 2 control in the two groups was compared. Drug understanding and medication compliance of the two groups were evaluated. Results The levels of fasting blood glucose, two hours postprandial blood glucose, glycated hemoglobin, total cholesterol, low density lipoprotein, systolic blood pressure, and diastolic blood pressure in the two groups after intervention were lower than those before intervention, and those in the study group were lower than those in the control group, the differences were statistically significant (all P < 0.05). After intervention, high density lipoprotein levels in both groups were higher than those before treatment, and those in the study group were higher than those in the control group, with statistical significance (all P < 0.05). The score of drug understanding degree in the study group was lower than that in the control group, and the score of drug compliance was higher than that in the control group, with statistical significance (all P < 0.05). Conclusion Chronic disease management led by clinical pharmacists can improve blood glucose, lipid and blood pressure and medication compliance in patients with diabetes mellitus type 2.
