跨理论模型下协同护理用于PCI老年冠心病患者自我管理能力及健康行为的影响

摘要目的探讨老年冠心病经皮冠脉介入术（PCI）患者采用跨理论模型（TTM）下协同护理方案的效果。方法选择2019年1月至2021年12月于三峡大学第三临床医学院接受PCI治疗的100例老年冠心病患者，根据随机数字表法将其分为参考组和研究组，各50例。参考组给予常规护理，研究组给予TTM下协同护理。随访3个月，比较两组自我管理能力、健康行为、术后出血及负性事件发生情况。结果随访3个月，两组健康促进生活方式调查问卷Ⅱ、自护能力测定量表各项评分均高于干预前，且研究组均高于参考组，差异有统计学意义（P＜0.05）；研究组内出血事件、负性事件发生率均低于参考组，差异有统计学意义（P＜0.05）。结论 TTM下协同护理能够促进老年冠心病行PCI患者自我管理能力的提高，并使其健康行为得以改善，有助于减少术后出血及负性事件的发生。

Abstract：Objective To investigate the effect of collaborative nursing under cross transtheoretical model (TTM) in elderly patients with coronary heart disease after percutaneous coronary intervention (PCI). Methods A total of 100 elderly patients with coronary heart disease who received PCI in the Third Clinical Medical College of China Three Gorges University from January 2019 to December 2021 were selected. According to the random number table method, the patients were divided into the reference group and the research group, with 50 cases in each group. The reference group was given routine nursing and the study group was given collaborative nursing under TTM. After three months, self-management ability, health behavior, postoperative bleeding, and the occurrence of negative events were compared between the two groups. Results After three months of follow-up, the scores of health promotion lifestyle profile Ⅱ and self-care ability scale in both groups were higher than before intervention, and the research group was higher than the reference group, the differences were statistically significant (P＜0.05). The incidence of internal bleeding events and negative events in the research group was lower than that in the reference group, the differences were statistically significant (P＜0.05). Conclusion Collaborative nursing under TTM can improve the self-management ability of elderly patients with coronary heart disease after PCI, improve their health behaviors, and help reduce the occurrence of postoperative bleeding and negative events.
