Diagnostic value of quantitative parameters of intravoxel incoherent motion imaging combined with ultrasound scoring method for antenatal placenta previa with placenta accreta

Abstract:Objective To explore the diagnostic value of quantitative parameters of intravoxel incoherent motion imaging combined with ultrasound scoring method for antenatal placenta previa with placenta accreta. Methods A total of 84 patients with antenatal placenta previa admitted to Nantong Maternity and Child Health Hospital of Jiangsu Province from March 2017 to September 2021 were selected as the research subjects. All subjects received antenatal ultrasound and MRI at 35-38 weeks of gestation. The quantitative parameters of intravoxel incoherent motion imaging and ultrasound score were analyzed, the results of postpartum pathology or surgery were taken as the gold standard, and the patients were divided into implantation group and non-implantation group according to whether they had placenta accreta. The quantitative parameters of intravoxel incoherent motion imaging and ultrasound scores, and pregnancy outcomes between the two groups were compared, receiver operating characteristic curve was used to evaluate the diagnostic efficacy of quantitative parameters of intravoxel incoherent motion imaging within voxel and ultrasound scores in the diagnosis of prenatal placenta previa with placenta accreta. Results Among the 84 patients with placenta previa, 31 patients(36.47%) had placenta accreta. The proportions of postpartum hemorrhage and Apgar score ≤ 7 at one minute after birth in implantation group were higher than those in non-implantation group (P < 0.05). The ultrasound score of implantation group was higher than that of non-implantation group, and hepatic arerial fracture (f value) was lower than that of non-implantation group (P < 0.05). The diagnostic sensitivity of single and combined f value and ultrasound score in the diagnosis of prenatal placenta previa with placental implantation were 77.42%, 80.65%, and 87.09% respectively, the specificity were 71.70%, 71.70%, and 75.47% respectively. Conclusion Quantitative parameters of intravoxel incoherent motion imaging and ultrasound scores have high clinical diagnostic efficacy for antenatal placenta previa with placenta accreta.
