Abstract With the rapid development of internet technology, mobile internet technology is rapidly changing the daily work of the medical and health industry, and playing an important role in the management of chronic diseases. This study will discuss how to effectively utilize advanced information technology, improve the utilization of existing medical resources, form a diabetes community health management system integrating the characteristics of traditional Chinese medicine, and explore the basic framework of designing a diabetes community health management system based on mobile terminal. The basic functions include login and registration, data acquisition, online service, information query, data analysis, intelligent reminder, and patient statistical analysis. Discusses the significance of designing a community diabetes management platform for diabetic patients and high-risk groups, grass-roots medical workers and the prevention and control mode of chronic diseases in traditional Chinese medicine community under the new technology. With the help of the extensive mass base of traditional Chinese medicine, the advantages of traditional Chinese medicine in the field of disease prevention and control are brought into play, and new management research directions are provided for the community diabetes prevention and control management in the new era.
