自拟固精益肾汤治疗肾病综合征的效果及对免疫功能的影响

摘要目的探讨自拟固精益肾汤治疗肾病综合征（NS）的效果及对免疫功能的影响。方法选取2016年2月—2019年12月就诊于安徽省芜湖市中医医院的NS患者82例，按随机数字表法将其分为对照组（41例）和研究组（41例）。对照组予以常规西药治疗，研究组在对照组的基础上联合自拟固精益肾汤治疗。比较两组患者疗效、中医症状积分、肾功能指标[24 h尿蛋白定量（24hPro）、血尿素氮（BUN）、肌酐（Scr）]、免疫功能[免疫球蛋白A（IgA）、免疫球蛋白M（IgM）、免疫球蛋白G（IgG）]及不良反应。结果治疗后，研究组临床总有效率高于对照组（P ＜ 0.05）。治疗后，两组的中医症状积分均较治疗前下降，且研究组低于对照组（P ＜ 0.05）。治疗后，两组24hPro、BUN、Scr均下降，且研究组低于对照组（P ＜ 0.05）。治疗后，两组IgA水平下降，且研究组低于对照组（P ＜ 0.05）；IgM、IgG水平升高，且研究组高于对照组（P ＜ 0.05）。两组不良反应发生率比较，差异无统计学意义（P ＞ 0.05）。结论自拟固精益肾汤治疗NS患者效果显著，可有效改善患者肾功能、免疫功能，且不升高不良反应发生率，具有较好的临床应用价值。

Abstract：Objective To explore the effect of self-made Gujing Yishen Decoction in the treatment of nephrotic syndrome (NS) and its influence on immune function. Methods A total of 82 cases of NS patients who were treated in Wuhu Traditional Chinese Medicine Hospital from February 2016 to December 2019 were selected. According to the random number table, the above patients were divided into the control group (41 cases) and the study group (41 cases). The control group was treated with conventional Western medicine, while the study group was treated with self-made Gujing Yishen Decoction on the basis of the control group. The curative effect, traditional Chinese medicine (TCM) syndrome score, renal function (24-hour urinary protein quantitative [24hPro], blood urea nitrogen [BUN], creatinine [Scr]),immune function (immunoglobulin A [IgA], immunoglobulin M [IgM], immunoglobulin G [IgG]) and adverse reactions were compared between the two groups. Results The total clinical effective rate of the study group was higher than the control group (P ＜ 0.05). After treatment, the symptom scores of the two groups were lower than that before treatment, and the study group was lower than the control group (P ＜ 0.05). After treatment, 24hPro, BUN and Scr levels were decreased in the two groups, and the study group was lower than those of the control group (P ＜ 0.05). After treatment, the IgA level decreased in the two groups, and the study group was lower than the control group (P ＜ 0.05), IgM and IgG increased in the two groups, and the study group was higher than those of the control group (P ＜ 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P ＞ 0.05). Conclusion Self-made Gujing Yishen Decoction has a significant effect on NS patients. It can effectively improve renal function and immune function without increasing the incidence of adverse reactions, with good clinical application value.
