Abstract:Objective To analyze the changes of interleukin-3 (IL-3), IL-6 and tumor necrosis factor α (TNF-α) levels before and after arthroscopic knee joint lateral support belt release and their value in predicting the prognosis of patients. Methods Retrospective analysis was performed on the clinical data of 123 patients with knee osteoarthritis (KOA) who underwent arthroscopic knee joint lateral support belt release in the Affiliated Zhoupu Hospital of Shanghai University of Medicine & Health Sciences from August 2016 to August 2018. Serum levels of IL-3, IL-6 and TNF-α were compared before and 14 days after surgery. Patients were followed up for one year after surgery and divided into poor prognosis group (30 cases) and good prognosis group(93 cases) according to their prognosis. The preoperative general clinical data and serum levels of IL-3, IL-6 and TNF-α were compared between the two groups, and the risk factors leading to poor prognosis were analyzed by logistic regression analysis. The receiver operating characteristic curve (ROC) was drawn to analyze the predictive value of risk factors for poor prognosis. Results The levels of IL-3, IL-6, and TNF-α were higher in the 14 days after surgery than before and the differences were statistically significant (all P < 0.05). There was no significant difference in gender, body mass index, family history, course of disease, hypertension, diabetes, cartilage injury, ligament injury, free body and preoperative VAS score between the two groups (all P > 0.05). The age of the poor prognosis group was older than that of the good prognosis group, the number of affected compartments was more than that of the good prognosis group, the preoperative Kellgren-Lawrence> 2 grade ratio and preoperative serum IL-3, IL-6 and TNF-α levels were higher than those of the good prognosis group, and the differences were statistically significant (all P < 0.05). Logistic regression analysis showed that age, number of affected compartments, preoperative Kellgren-Lawrence> 2 grade, preoperative serum IL-3, IL-6 and TNF-α levels were all risk factors leading to poor prognosis (P < 0.05). ROC results indicated that the optimal cutoff value for IL-3, IL-6 and TNF-α for poor prognosis were 248.35 pg/mL, 174.61 pg/mL and 1.96 ng/mL, and the AUC was 0.752, 0.774 and 0.704, respectively. Conclusion The levels of serum IL-3, IL-6 and TNF-α increase in KOA patients after arthroscopic knee joint lateral support belt release, and the preoperative serum levels of these three factors have certain value in predicting the prognosis of the disease.
