Abstract:Objective To investigate the risk factors of incontinence-associated dermatitis (IAD) in patients in surgical intensive care unit (ICU) and to establish a prediction model. Methods A total of 116 patients were collected by continuous sampling from July 2018 to April 2019 in ICU of Xuanwu Hospital Capital Medical University. They were divided into non-IAD group (93 cases) and IAD group (23 cases) according to the presence or absence of IAD. The influencing factors of IAD were analyzed by logistic stepwise regression analysis, while construct a prediction model, and evaluate the effect. Results The incidence of IAD in surgical ICU patients was 19.83%. The main influencing factors included PAT risk stratification, Barthel index, acute physiology and chronic health evaluation (APACHEⅡ), Braden score and age (P < 0.05). Braden score (OR = 0.593, 95%CI: 0.415-0.846, P < 0.01), PAT risk stratification (OR = 8.755, 95%CI: 1.038-73.821, P < 0.05), age>70 years (OR = 4.474, 95%CI: 1.112-17.993, P < 0.05) were independent risk factors. A risk prediction equation was established based on multivariate logistic regression: logit(P) = -1.848-0.523X1+2.170X2+1.498X3, and the Hosmer-Lemeshow was used to test the goodness of fit of the regression equation (P = 0.237). The area under the ROC curve was 0.810 (95%CI: 0.704-0.916, P < 0.01). Conclusion Low Braden score, high risk of PAT assessment, and age>70 years old are independent risk factors for IAD in surgical ICU patients. Establishing a predictive model of IAD is helpful for early identification of high-risk patients with IAD.
