Expression changes and clinical significance of TLR-4 and IL-1β in serum and knee joint fluid of patients with knee osteoarthritis

Abstract:Objective To investigate the expression changes and clinical significance of Toll-like receptor-4 (TLR-4) and interleukin-1β (IL-1β) in serum and knee joint fluid of patients with knee osteoarthritis (KOA). Methods From June 2017 to December 2018, 220 cases with KOA who admitted in Department of Orthopedics, the Third Affiliated Hospital of Inner Mongolia Medical University (“our hospital” for short) were selected as observation group. According to the severity of KOA, they were divided into mild subgroup (n = 98), moderate subgroup (n = 80) and severe subgroup (n = 42). In addition, 60 cases of volunteers were selected for physical examination in our hospital of health center medical as control group. The levels of TLR-4 and IL-1β in serum and knee joint fluid were detected by enzyme-linked immunosorbent assay. The correlation between TLR-4, IL-1β levels and KOA severity were analyzed and their correlation were analyzed. Results The levels of TLR-4 and IL-1β in serum and knee joint fluid of observation group were significantly higher than those of control group, and the differences were statistically significant (all P < 0.05). The levels of TLR-4 and IL-1β in severe subgroup were significantly increased than those of moderate subgroup and mild subgroup, compared with mild subgroup, the levels of TLR-4 and IL-1β in moderate subgroup were significantly increased, and the differences were statistically significant (all P < 0.05). Serum and knee joint fluid TLR-4 and IL-1β levels were positively correlated with KOA severity (r = 0.706, 0.729, 0.741, 0.715, all P < 0.05). Serum TLR-4 level was positively correlated with IL-1β level (r = 0.720, P < 0.05). The level of TLR-4 in knee joint fluid was positively correlated with IL-1 level (r = 0.736, P < 0.05). Conclusion The levels of TLR-4 and IL-1β in serum and knee joint fluid of KOA patients unusually increase, which is closely related with the severity of KOA. It can be used for clinical prediction, diagnosis and treatment of KOA.
