Abstract:Objective To investigate comments and intervention analysis of unreasonable outpatient prescription pharmacists before dispensing. Methods 192 300 prescriptions were collected from January to June 2018 in Ma′anshan People′s Hospital of Anhui Province. Pareto chart was used to analyze the main types of irrational prescriptions, the causes and formulate intervention strategies were analyzed. Prescription intervention was conducted from July to December 2018 to observe the change of the incidence of irrational prescriptions. Results There were 14 042 irrational prescriptions among 192 300 prescriptions before intervention, accounting for 7.30%. Pareto chart showed that the main types of irrational prescription are no diagnosis or incomplete diagnosis, unsuitable usage and dosage, and unsuitable drug selection. There were 8245 irrational prescriptions among 196 310 prescriptions after intervention, accounting for 4.20% after intervention. The incidence of irrational prescriptions decreased significantly (P < 0.05). Conclusion The intervention of pharmacists before prescription dispensing can effectively reduce the incidence of irrational prescriptions and improve the level of pharmaceutical care.
