意外胆囊癌的影像学特征及误漏诊分析

摘要目的讨论意外胆囊癌的影像学特征，分析误漏诊原因并提出防范措施，减少意外胆囊癌的发生。方法回顾性分析2016年1月到2018年12月南京鼓楼医院收治的12例意外胆囊癌患者的术前CT、MRI图像，并与病理结果对比。结果 12例患者术后病理均确诊胆囊癌，影像学检查8例误诊，其中7例为腔内型癌，1例为壁厚型癌；4例漏诊，均为壁厚型癌。结论壁厚型胆囊癌容易漏诊，腔内型胆囊癌易误诊，加强对胆囊病变的影像特征认识，规范检查技术，可以提高诊断意外胆囊癌的准确率。

Abstract：Objective To summarize imaging fearures of unsuspected gallbladder cancer （UGC）, analyze causes of missed diagnosis or misdiagnosis and provide prevention method, in order to reduce the incidence of UGC. Methods The preoperative CT and MRI imagings of 12 UGC patients who treated in the Affiliated Drum Tower Hospital of Nanjing Medical University from January 2016 to December 2018 were retrospectively analyzed and compared with the pathological results. Results The postoperative pathology diagnosis of the 12 cases were confirmed as gallbladder cancer. There were 8 cases out of 12 cases were misdiagnosed by imaging examinations, for the intracavitary type, 7 cases were nodules cancer, and 1 case was infiltration cancer. And 4 cases out of 12 cases were miss diagnosis, the 4 cases were all linfiltration cancer and thick-walled carcinomas. Conclusion The linfiltration gallbladder cancer is easy to be missed, and the nodules gallbladder cancer is easy to be misdiagnosed. Improved understanding on the imaging signs of gallbladder diseases and used standard imaging examination techniques, the preoperative diagnosis rate of UGC will be improved.
