Study on the implementation effect of medical service price reform from the perspective of patients

Abstract:Objective To explore the effect of medical service price reform and the factors that affect the patients′ evaluation of medical expenses. Methods From September 2017 to February 2018, one public hospital was selected from the five regions of Beijing, Tianjin, Hebei, Shandong and Liaoning respectively, patients of five outpatients and inpatients were investigated. Literature analysis method and expert interview method were used to design questionnaires on the formation mechanism of dynamic adjustment of medical service prices. The reform effect of was analyzed based on the three dimensions such as characteristics of individuals, the status quo of the current medical service price policy and the implementation effect of the current medical service price policy. Results The total price of medical services in the five regions of Beijing, Tianjin, Hebei, Shandong and Liaoning was 3.861 points. Patients rated the cost of surgery and examination higher, they scored 4.097 and 4.011 respectively. Multiple linear regression analysis showed that age and whether the patient had commercial medical insurance affected the patient′s evaluation of comprehensive medical expenses (P < 0.05). Patients′ evaluation of the comprehensive effect after the reform was affected by the level of the medical institutions they visited recently and whether they still had commercial medical insurance (P < 0.05). The different gender and age paid attention to the distribution of medical policy, and the difference was statistically significant (P < 0.05). At different gender levels, there was a statistically significant difference in the distribution of access to medical service price reform policies for respondents of different age groups (P < 0.05). Conclusion The implementation effect of medical service price reform is good, but it still needs to expand the publicity of medical service price reform policy, optimize the price structure of medical service items, and increase the coverage of commercial medical insurance to relieve the medical burden of patients.
