黄芪注射液联合大量丙种球蛋白治疗小儿急性重症病毒性心肌炎的效果观察

摘要目的探讨黄芪注射液联合大量丙种球蛋白治疗小儿急性重症病毒性心肌炎的效果。方法选取2016年1月～2019年1月于解放军联勤保障部队第九八八医院小儿急性重症病毒性心肌炎患儿78例，按照随机数字表法分为对照组（n = 39）和观察组（n = 39）。对照组接受常规治疗，观察组同时接受黄芪注射液联合大量丙种球蛋白治疗，比较两组临床疗效、治疗前后炎性因子及心肌酶水平、安全性。结果观察组总有效率明显高于对照组（P ＜ 0.05）。治疗后，两组白细胞介素8、白细胞介素17及肿瘤坏死因子-α水平均明显低于治疗前，且观察组明显低于对照组（P ＜ 0.01）。治疗后，观察组血清肌酸激酶同工酶、乳酸脱氢酶及肌酸激酶水平均明显低于治疗前，且观察组明显低于对照组（P ＜ 0.01）。两组患儿均未见明显肝肾功能异常等药物不良反应。结论黄芪注射液联合大量丙种球蛋白治疗小儿急性重症病毒性心肌炎的效果明确，值得推广应用。

Abstract：Objective To explore the effect of Astragalus Injection combined with large amount of Gamma Globulin in the treatment of acute severe viral myocarditis in children. Methods A total of 78 children with acute severe viral myocarditis in the 988th Hospital of PLA Joint Logistics Support Force from January 2016 to January 2019 were selected and divided into control group (n = 39) and observation group (n = 39) according to random number table method. The control group received routine treatment, while the observation group received Astragalus Injection combined with a large amount of Gamma Globulin. The clinical efficacy, inflammatory factors, myocardial enzymes levels and safety of two groups were compared before and after treatment. Results Total effective rate of observation group was significantly higher than that of control group (P < 0.05). After treatment, levels of IL-8, IL-17 and TNF-α in two groups were significantly lower than those before treatment, and those in observation group were significantly lower than the control group (P < 0.01). After treatment, CK-MB, LDH and CK levels in observation group were significantly lower than those before treatment, and those in observation group were significantly lower than the control group (P < 0.01). There were no obvious adverse drug reactions such as abnormal liver and kidney function in both groups. Conclusion Astragalus Injection combined with a large amount of Gamma Globulin in the treatment of acute severe viral myocarditis in children has a definite effect, which is worthy of popularization and application.
