知信行模式在放化疗性脊髓损伤患者护理中的应用价值

摘要目的探究与分析知信行模式在放化疗性脊髓损伤患者护理中的应用效果。方法选取中国医科大学附属盛京医院2014年6月~2016年6月收治的90例放化疗性脊髓损伤患者，采取随机数字表法分为对照组与观察组，每组各45例。对照组仅给予常规护理，观察组在对照组基础上加用知信行模式，两组均连续护理1个月。对比两组疼痛评分、功能独立性评价量表评分、焦虑抑郁情绪及生活质量。结果与护理前相比，两组护理后疼痛数字评分法（NRS）评分降低，功能独立性评定量表（FIM）评分升高，观察组改善更加显著，差异有统计学意义（P < 0.05）。与护理前相比，两组护理后焦虑自评量表（SAS）及抑郁自评量表（SDS）评分均明显降低，且护理后观察组评分低于对照组，差异有统计学意义（P < 0.05）。与护理前相比，两组护理后躯体功能、角色功能、认知功能、情绪功能及社会功能评分均升高，且护理后观察组评分高于对照组，差异有统计学意义（P < 0.05）。结论在常规护理基础上加用知信行模式应用于放化疗性脊髓损伤患者护理中能够改善缓解疼痛，促进恢复功能，缓解焦虑抑郁情绪，提高生活质量。

Abstract：Objective To explore and analyze the application effect of knowledge, attitude, practice model in nursing of patients with radiation and chemotherapy of spinal cord injury. Methods From June 2014 to June 2016, 90 cases with radiation and chemotherapy of spinal cord injury treated in Shengjing Hospital of China Medical University were selected. All the patients divided into the control group and observation group according to the random number table, with 45 cases in each group. The patients in the control group were given to routine nursing only, and the patients in the observation group were given to knowledge, attitude, practice model based on the control group. All the patients were treated for one month. The pain score, functional independence evaluation questionnaire score, anxiety, depression and quality of life of two groups were compared. Results The NRS score of two groups after nursing were lower than before, and FIM score was increased, the improvement of observation group was more significant than that of the control group after nursing nursing, with statistically significant difference (P < 0.05). The SAS and SDS scores of the two groups after nursing were significantly lower than before, the decrease of scores in observation group were more remarkable than that in the control group after nursing, with statistically significant difference (P < 0.05). The physical function, role function, cognitive function, emotional function and social function score of the two groups after nursing were increased, and the improvement of observation group was more significant than that of the control group, with statistically significant difference (P < 0.05). Conclusion The application of knowledge, attitude, practice model treated in the patients with radiation and chemotherapy of spinal cord injury based on nursing can improve pain relief, promote the functional recovery, reduce anxiety depression, and improve the quality of life.
