Evaluation on the cognitive function of patients with chronic obstructive pulmonary disease and analysis on its influencing factors

Abstract:Objective To evaluate the cognitive function of patients with chronic obstructive pulmonary disease (COPD), and to analyze its influencing factors. Methods From February 2015 to July 2016, 68 patients with COPD admitted to Xinqiao Hospital, Third Military Medical University ("our hospital" for short) were selected as COPD group, and they were divided into mild-moderate group (37 cases) and severe group (31 cases) according to the severity of illness, 28 healthy persons in our hospital at the same time were selected as normal control group. The cognitive function of subjects were evaluated by mini-mental state examination (MMSE), the indexes of pulmonary function and arterial blood gas were detected respectively by pulmonary function detector and automatic blood gas analyzer. Results Compared with normal control group, the scores of MMSE and the indexes of pulmonary function in mild-moderate group and severe group decreased, the differences were all highly statistically significant (P < 0.01). The indexes of arterial blood gas PaO2, SaO2 in mild-moderate group were all lower than those of normal control group, the differences were all highly statistically significant (P < 0.01), while there was no significant difference of PaCO2 between mild-moderate group and normal control group (P > 0.05). The indexes of arterial blood gas PaO2, SaO2 in severe group were all lower than those of normal control group (P < 0.01), and PaCO2 was higher than that of normal control group, the difference was highly statistically significant (P < 0.01). Compared with mild-moderate group, the scores of MMSE and the indexes of pulmonary function in severe group decreased, the PaCO2 increased, the differences were all highly statistically significant (P < 0.01), there were no statistically significant differences of PaO2 and SaO2 between the two groups (P > 0.05). Pearson analysis showed that, the scores of MMSE in mild-moderate group were significantly positively correlated with PaO2 (r = 0.46, P < 0.05), the scores of MMSE in severe group were significantly positively correlated with FEV1 and PaO2 (r = 0.45, 0.40, P < 0.05). Conclusion The patients with COPD has obvious cognitive impairment, and the degree of cognitive impairment is correlated with severity of illness, among which, FEV1 and PaO2 are the important influencing factors of the occurrence of cognitive impairment in patients with COPD.
