Abstract:Objective To investigate the changes of toll-like receptor 4 (TLR4) and trmor necrosis factor-α (TNF-α) in peripheral blood of patients with acute myocardial infarction (AMI) and their clinical significance. Methods From June 2015 to March 2018, 103 AMI patients admitted to Yibin Second People′s Hospital of Sichuan Province were selected as the research group. According to the complications, the patients were divided into uncomplicated group (29 cases), complication group (55 cases) and death group (19 cases). According to the treatment effect, the patients were divided into the cured group (43 cases), the improved group (40 cases) and the ineffective group (20 cases). In addition, 51 patients with chest pain who participated in physical examination in the same period were selected as the control group. Meanwhile, the effects of TLR4, TNF-α and Gensini scores in peripheral blood of subjects in the study group and the control group on AMI subgroup and efficacy were observed. In addition, the correlation between TLR4 and serum TNF-α and Gensini scores in peripheral blood were analyzed. Results TLR4, TNF-α and Gensini scores in the peripheral blood of the study group were significantly higher than those of the control group (P < 0.05); The scores of TLR4, TNF-α and Gensini in the peripheral blood of patients in the complication group and the death group were significantly higher than those in the non-complication group, and the scores of TLR4, TNF-α and Gensini in the peripheral blood of patients in the death group were significantly higher than those in the complication group, with statistically significant differences (P < 0.05). Compared with the treatment group, the scores of TLR4, TNF-α and Gensini in peripheral blood of patients in the improved group and the ineffective group were significantly higher than those in the improved group (P < 0.05). TLR4 in peripheral blood of AMI patients was positively correlated with serum TNF-α and Gensini score (r = 0.751, 0.703, P < 0.05), while serum TNF-α was positively correlated with Gensini score (r = 0.729, P < 0.05). Conclusion TLR4 and TNF-α are involved in the occurrence and development of AMI, and can also evaluate the clinical therapeutic effect and provide new therapeutic targets for the clinical treatment of AMI.
