头部自重后伸牵引配合针刀治疗颈源性头痛的效果

摘要目的探讨头部自重后伸牵引配合针刀治疗颈源性头痛的临床效果。方法选择2016年3月～2017年3月就诊于承德医学院附属医院的颈源性头痛患者120例，采用抛硬币法随机分为观察组（60例）和对照组（60例）。观察组利用床旁头部自重后伸牵引的方法配合针刀治疗，对照组采用针刀治疗，疗程4周。治疗前后均行颈椎侧位的X线摄片，测量颈曲值，观察治疗前后颈椎曲度的改变情况。治疗前、治疗后4周及治疗后3个月，分别行疼痛视觉模拟评分（VAS），观察两组的临床疗效，并进行统计分析。结果治疗后4周，观察组总有效率明显高于对照组，差异有高度统计学意义（P ＜ 0.01）。治疗后3个月，观察组颈源性头痛复发率（3.33%）明显低于对照组（16.67%）（P ＜ 0.01）。两组治疗后4周、3个月VAS评分均明显低于治疗前，且观察组VAS评分明显低于对照组，差异有高度统计学意义（P ＜ 0.01）。治疗后4周，两组颈曲值均明显高于治疗前，且观察组高于对照组（P ＜ 0.01）。治疗后4周，观察组颈曲总改变率明显高于对照组，差异有高度统计学意义（P ＜ 0.01）。结论头部自重后伸牵引配合针刀治疗颈源性头痛效果显著，复发率低，值得临床推广。

Abstract：Objective To investigate the clinical efficacy of head self-weight backward traction with acupotomy in the treatment of cervicogenic headache. Methods One hundred and twenty cases of patients with cervicogenic headache admitted to Affiliated Hospital of Chengde Medical University from March 2016 to March 2017 were selected and randomly divided into observation group (60 cases) and control group (60 cases) by fliping coins. The observation group was given the treatment of bedside head self-weight backward traction with acupotomy, and the control group was given acupotomy, the course was 4 weeks. Before and after treatment, X-ray examination of lateral cervical spine was taken, and the cervical curvature values were measured to observe the changes of cervical curvature before and after treatment. Before treatment and after treatment for 4 weeks, 3 months, the visual analogue scale (VAS) was performed respectively, the clinical efficacy of the two groups was observed and statistical analysis was carried out. Results After treatment for 4 weeks, the total effective rate of the observation group was significantly higher than that of the control group, the difference was highly statistically significant (P ＜ 0.01). After treatment for 3 months, the recurrence rate of cervicogenic headache of the observation group (3.33%) was lower than that of the control group (16.67%) (P ＜ 0.01). After treatment for 4 weeks, 3 months, the VAS scores of the two groups were all lower than those before treatment, which of the observation group were lower than those of the control group, the differences were highly statistically significant (P ＜ 0.01). After treatment for 4 weeks, the cervical curvature values of the two groups were higher than those before treatment, which of the observation group were higher than those of the control group (P ＜ 0.01). After treatment for 4 weeks, the total change rate of cervical curvature in the observation group was significantly higher than that in the control group, the difference was highly statistically significant (P ＜ 0.01). Conclusion The effect of head self-weight backward traction with acupotomy in the treatment of cervicogenic headache is significant, and the recurrence rate is low, which is worthy of clinical promotion.
