应用甲氨蝶呤与来氟米特联合手指操缓解活动性类风湿关节炎患者晨僵症状的效果

摘要目的探讨甲氨蝶呤与来氟米特联合手指操缓解活动性类风湿关节炎患者晨僵症状的效果。方法回顾性分析湖北医药学院附属东风医院2015年1月～2017年1月收治的活动性类风湿关节炎患者临床资料98例，根据治疗方案的不同，将其分为对照组和观察组，每组各49例。对照组采用甲氨蝶呤联合来氟米特进行治疗，观察组在对照组基础上加用手指操进行治疗。比较两组患者临床疗效、手指和腕部关节疼痛程度、晨僵时间、手功能改善情况及不良反应。结果观察组患者总有效率（95.91%）明显高于对照组（83.67%），差异有统计学意义（P ＜ 0.05）。治疗后两组患者VAS评分、晨僵时间及SOFI评分均低于治疗前，差异有统计学意义（P ＜ 0.05）。治疗后观察组上述各评分均明显低于对照组，差异均有统计学意义（P ＜ 0.05）。两组患者在治疗过程中均未出现严重不良反应。结论应用甲氨蝶呤与来氟米特联合手指操治疗活动性类风湿关节炎临床疗效佳，可有效改善患者晨僵症状，值得临床推广。

Abstract：

Objective To investigate the effect of methotrexate and leflunomide combined with finger exercises on relieving morning numb in patients with active rheumatoid arthritis. Methods From January 2015 to January 2017, 98 clinical data of patients with active rheumatoid arthritis treated in Dongfeng Hospital Affiliated to Hubei University of Medicine were retrospectively analyzed and divided into the control group and the observation group, with 49 cases in each group according to the treatment methods. The control group was given methotrexate combined with leflunomide and the observation group was given finger exercises on the basis of the control group. The clinical efficacy, finger and wrist pain, morning stiffness, hand function improvement and adverse reaction in the two groups were compared. Results The total curative rate in the observation group (95.91%) was significantly higher than that of control group (83.67%), with statistically significant difference (P ＜ 0.05). After treatment, the VAS score, morning stiffness time and signals of functional impairment score (SOFI) in the two groups were all lower than those of before treatment, the indicators above in the observation group were all lower than those of control group, with statistically significant differences (P ＜ 0.05). There were no severe adverse reactions in the two groups during the treatment. Conclusion Methotrexate and leflunomide combined with finger exercises in the treatment of patients with active rheumatoid arthritis is good. It can effectively improve the stiffness symptoms. It is worthy of clinical promotion.
