Abstract:Objective To systematically evaluate the relevance between the prophylactic use of antimicrobial drugs and the incision infection after episiotomy, and to guide rational medication in clinic. Methods PubMed, Embase, Cochrane Library, Chinese Biomedical Literature Database, Chinese Academic Journal Full-text Database and Wanfang Database were used for searching all relevant literatures since the libraries were built up to December 2017, and the studies of correlation between prophylactic use of antimicrobial drugs and incision infection after episiotomy were collected. RevMan 5.3 software was used for Meta-analysis. Results A total of 15 articles were included with 4355 patients. Meta-analysis showed that prophylactic use of antimicrobial drugs was associated with the incision infection after episiotomy[OR = 0.50, 95%CI (0.35, 0.71), P = 0.0001]. Subgroup analysis about the modes of action of antimicrobial drugs suggested that the use of focal irrigation had a relevance to the incision infection after episiotomy [OR = 0.25, 95%CI (0.12, 0.53), P = 0.0003], while there was no significant association between systemic application and incision infection [OR = 0.78, 95%CI (0.48, 1.26), P = 0.31]. Conclusion Prophylactic use of antimicrobial drugs may reduce the incidence of incision infections after episiotomy, especially in focal irrigation applications. However, due to the limited number of documents and sample size, large-sample, multi-center and well-designed clinical trials are still needed to confirm and improve the conclusions of this study.
