Investigation and analysis on off-label drug use in Pediatric Outpatient of General Hospital of Ningxia Medical University in 2017

Abstract:Objective To investigate and analyze the situation on off-label drug use for children in Pediatric Outpatient of General Hospital of Ningxia Medical University ("our hospital" for short), in order to provide a reasonable guidence for children medication in clinic. Methods From January to December 2017, Pediatric Outpatient drug prescriptions were selected randomly in our hospital. According to the medicine dispensatory, whether medication orders were over the package insert, the types and medication situation at different ages of off-label drug use, and the correlation between off-label drug use and basic medicine were analyzed. Results A total of 2640 medical prescriptions were selected from the Outpatient Department of Pediatrics, 4293 prescription medication advices were evaluated. According to the prescription and medication orders, the incidence rate of off-label drug use was 11.14% and 7.29% respectively. The main types of off-label drug use were the dosage and frequency of drug delivery, the incidence rates were 4.52% and 1.51% respectively, and the constituent ratios were 61.98% and 20.77% respectively, followed by the indications, the incidence of which was 0.96%, and the constituent ratio was 13.10%. Few references were related with children's medication information and contraindications. According to medication orders, the incidence rate of off-label drug use of the first two patient ages were neonatal (100.00%) and pre-school age (11.24%). The top four drugs of off-label drug use were dermatological drugs (20.00%), vitamin (16.93%), liver protection (13.64%) and digestive system and metabolic drugs (7.96%). Conclusion Due to the phenomenon of off-label drug use in Pediatric Outpatient in our hospital, the clinical pharmacists should pay close attention to the phenomenon and intervene rationally.
