慢性鼻-鼻窦炎鼻息肉患者内镜手术前后VEGF、Eotaxin、黏蛋白MUC5AC表达的临床意义

摘要目的探究慢性鼻-鼻窦炎鼻息肉患者内镜手术前后嗜酸性粒细胞趋化因子（Eotaxin）、黏蛋白MUC5AC、血管内皮生长因子（VEGF）的表达变化及临床意义。方法选取2013年2月～2016年12月上海市奉贤区中心医院62例慢性鼻-鼻窦炎鼻息肉内镜手术患者为观察组，并选取同期因鼻中隔偏曲住院行鼻中隔矫正术患者62例为对照组。观察两组HE染色情况，观察比较两组手术前后嗜酸性粒细胞浸润、Eotaxin、黏蛋白MUC5AC、VEGF表达情况。结果观察组术后嗜酸性粒细胞明显少于术前，差异有高度统计学意义（P ＜ 0.01）。与对照组比较，观察组手术前后嗜酸性粒细胞计数均较高，差异有高度统计学意义（P ＜ 0.01）。观察组术后Eotaxin、黏蛋白MUC5AC、VEGF表达面积百分率明显低于术前，差异有高度统计学意义（P ＜ 0.01）。与对照组比较，观察组手术前后Eotaxin、黏蛋白MUC5AC、VEGF表达面积百分率均较高，差异有高度统计学意义（P ＜ 0.01）。结论慢性鼻-鼻窦炎鼻息肉患者鼻黏膜组织Eotaxin、黏蛋白MUC5AC、VEGF均表现为过度表达，可能与病情变化有关。

Abstract：Objective To investigate the level change and clinical significance of the expressions of eosinophils chemokines (Eotaxin), mucin MUC5AC and vascular endothelial growth factor (VEGF) in patients with chronic rhinosinusitis and nasal polyps before and after endoscopic surgery. Methods From February 2013 to December 2016 in Shanghai Fengxian District Central Hospital, the 62 cases with chronic sinusitis and nasal polyps treated with endoscopic surgery were selected as the observation group, and 62 cases of hospitalized patients with deviation of nasal septum treated with septum rectifying operntion were selected as the control group. The HE staining result, the infiltration of eosinophils, the expressions of Eotaxin, mucin MUC5AC and VEGF were observed and compared between the two groups before and after operation. Results The eosinophil count in the observation group was significantly less than that before operation, and the difference was statistically significant (P ＜ 0.01). Compared with the control group, the eosinophil count was higher in the observation group before and after operation, and the difference was statistically significant (P ＜ 0.01). The expression percentage of Eotaxin, mucin MUC5AC and VEGF was significantly lower in the observation group than those before operation, and the differences were statistically significant (P ＜ 0.01). Compared with the control group, the percentages of Eotaxin, mucin, MUC5AC and VEGF expression were higher in the observation group before and after the operation, and the differences were statistically significant (P ＜ 0.01). Conclusion The Eotaxin, mucin, MUC5AC and VEGF in the chronic rhinosinusitis and nasal polyps patient are over expressed, and they are closely related to the changes of disease condition.
